Dillon Precision (the reloading machine people) sell two leather holsters for the Makarov. One is a horsehide IWB with a metal belt clip, and the other is a "pancake" type belt holster. I have an example of each - the IWB for a Walther, and the "pancake" for a S&W 908 - and like 'em both.

Personally I don't like the nylon Uncle Mikes holsters, as they don't stay open very well for reholstering, and they don't fit the gun very well so they cause extra finish wear from the gun slopping around inside.

Oh, forgot the web site:

Fobus makes a respestable holster for about $20.
Or, if you want a really nice Kydex holster... I can recommend Blade-Tech. Blade-Tech's prices are about 1/2 that if the Makarov pistol...Well, I guess if you really, really want one... this would be the holster for you!
