zeroing scope

Picked up an new Bushy varminter and had a Bushnell elite 4200 mounted with Millet angle-loc rings by a gunsmith. Went to the range, and despite repeated adjustments, it still wont' come within five inches of where i'm pointing it. Suggestions?

Am not familiar with this scope or mount or rings . Are you tight with all of the above ?

Is it off left - right ? Up - Down ?
