What I don't understand is why would people do drugs like this knowing what the effects would be. Are they so hard up for a high they literally don't care if their flesh rots off and they die, do they not believe it and get hooked and then it's too late?
It's sad someones life is so empty they have to resort to this to feel something.

It' not just being so hooked that they'll do risk anything for a hit. It's also the depresion and fatalism that comes with leading the lifestyle. When they're doing something daily that might kill them on the next dose they don't think about things that might kill them years down the road.
It's not even living day to day. It's living shot to shot.
