Not to mention the times when a nuke was accidentally salvoed off the bomber it was mounted in. Imagine a group of general officers standing near a bomber and they hear a loud thunk and look over at the bomber to see said just loaded nuke laying on the tarmac. Absolute silence. Finally someone goes over to the bomb and listens to it closely, then gestures for the airmen to come and load it back in the ac properly. After loading the bomb once again, one of the officers asks the guilty party if he regularly salvoed nuclear weapons on the tarmac.
"And how we burned in the camps later thinking, what would things have been like, if every security operative, when he went out at night to make an arrest, had been uncertain, whether he would return alive and had to say good-bye to his family?"
Lethal Arrogance is a good book to read. IIRC they detail a lot of the mishaps. I think it's Georgia or south Carolina that escaped being nuked because of just one on the interlocks of 6 preventing detonation. One of the two bombs was never found believed to be off the coast. Could be "in mortal hands" can't remember which book, but they are both excellent reads about our susceptibility making mistakes a human beings. "Command and control" is a damn good read about the Titan Missie silo disaster in Arkansas.
