For the last two summers I was living in California and the smoke from the fires was indescribable. It basically would irritate my ear canals and create fluid thus giving me vertigo.
It took a handful of visits to a doctor to figure it out and it wasn't a fast fix. There are so many things that can give you vertigo I guess but they usually start with the ears when finding a cause.
I did notice that taking meclizine helped me though.
I get it when I hike in high places along cliffs or drive on high roads with no guardrails like 550 from Silverton to Ouray.
I get vertigo occasionally. From going to medical they said, and I shit you not, that sometimes there are what they described as rocks in your ears that sometimes will hit your balance apparatus that can give you the feeling of vertigo.
Have you been congested lately? The two types of vertigo I am most familiar with are choloric and alternobaric. Choloric vertigo is when you have varying temperatures on your eardrums, which could happen if you jump in the shower and warm water goes in one ear and not the other. Alternobaric is having varying pressures being exerted on your eardrum, which could happen if you are congested and are sneezing/coughing a lot and get a fullness or muffled sound in one ear. Both conditions happen in the dive field quite a bit.
What ever it is, I hope its minor and goes away. If it is persistence, then your friend definitely should get it checked.
I've had or more or less constant for a few years, it sucks. I've had it treated a number of times and it always comes back. I think I have an ear that doesn't drain very well. They treated it like an ear infection a few times with different antibiotics and that didn't help, they gave me steroids which made me feel really weird and only providedtemporary relief. I think I have to just live with it.
Have your friend check his/her blood pressure. Low BP can cause dizziness every time you stand up from either a sitting position or a reclining (horizontal) position.
My wife's blood pressure runs low and she has that problem.
In a diabetic, low blood sugar can cause it, too.

I had this a few years ago.
The tests involved wearing a set of goggles while lights were projected inside and I was instructed to follow the light with my eyes.
I was also told by the specialist that tiny "rocks" in the inner ear could shift and cause this.
The answer was to do a certain set of head and eye movement exercises a few times a day.
I was told that if I did them for six weeks that would be a permanent "cure".

Yes it did.
One set was to keep your eyes centered in your head and rotate your head from left to right and back.
This is like just looking from far right to far left and back without moving your eyes.
The other was to center your eyes on an object and rotate your head from right to left keeping the eyes on the object.
As you progress with each exercise speed up the movement.
I think you do each 10 head movements then a short rest and do another "set".
I forgot how many sets do do.
The doctor said that if you start to have vertigo put your thumb up about 8 to 12 inches in front of your face and concentrate your eyes on it and hold until it stops.

I have always had severe vertigo. I have trouble walking up to the window of my daughter's 25th floor apartment unless I move really slow and adjust as I go. I have trouble walking across a bridge over any river, and when driving a car in the mountains, if there was a cliff to the right as we drove, I tended to crowd the center line. The Grand Canyon was a bear!
On the other hand I am a pilot and have no problems looking down from the windows of an aircraft.
